Procedures for Obtaining Employment Certificates

 

Minors (under age 18) require an employment certificate to obtain employment. Employment certificate applications consist of form AT-16, the Physical Fitness Certification and form AT-17, the Application for Employment Certificate.

These forms can be obtained at the Middle School Student Support Services Office, on the Middle School’s website under Student Support Services, or they can also be found on the New York State Education website at http://www.p12.nysed.gov/sss/pps/workingpapers/. The forms online are fillable but then they must be printed and turned into the Student Support Services office.

 

  1. Part I of The Application for Employment Certificate (form AT-17) must be completed. If the student attends Iroquois, a copy of their birth certificate is on file for Part II. If the student does not attend Iroquois, they will need to provide a copy of their birth certificate.

 

  1. The Physical Fitness Certificate (form AT-16 and part III of AT-17) must be filled out by the student’s family doctor or the school nurse can complete it if the student has had a physical within the past year.

 

  1. Bring the completed forms to the Student Support Services office for processing. The student will be notified when the certificate is ready and must personally come to pick it up so they can sign the certificate in front of the issuing official.

 

Blue Certificate – 14 and 15 years of age

  • When a student turns 16 the paperwork will need to be completed again.

Green Certificate – 16 and 17 years of age

 

The employer retains the certificate while the student is employed. When the student leaves they should ask the employer for the certificate back.

If for some reason another copy is needed, ask the SSS for a duplicate copy. The paperwork would not need to be redone.
